• Transfer Certificate (if applicable)
• Copy of Aadhar card (Child, Father, Mother, Guardian)
• Last report card (if applicable)
• Declaration of medical history (if any) with supportive documents to be attached in a separate sheet
• Two passport size photographs (child)
